Important Points

• While Creative Ways will verify client benefits with the insurance provider, it is ultimately the responsibility of the client to be well-informed about their benefits. We encourage you to contact your provider to understand any fees you will be expected to pay for a deductible or co-pay.

• If your insurance provider requires that you pay a deductible or co-pay, Creative Ways will ask that you provide a credit card which will be billed monthly.

• If you are a self-paying client, Creative Ways will ask that you provide a credit card which will be billed monthly.  Please feel free to discuss your situation with Teresa, Donna, or your particular therapist, and we will do our best to offer a sliding scale to meet your particular needs.

• If you have insurance that Creative Ways does not accept, you may contact your provider to inquire about out-of-network coverage. If your insurance plan will cover out-of-network providers, then you will be required to pay  Creative Ways self-pay rate out-of-pocket for each therapy session. You will then submit a monthly receipt of services to request reimbursement, or partial reimbursement from your insurance company. If requested, Creative Ways can provide a monthly receipt of services.
